Description of Activities
I, the undersigned, understand that I (or my minor child/ward named above) will be participating in baseball-related activities, including but not limited to the use of an outdoor batting cage and/or private baseball lessons provided by The Front Yard Baseball (hereinafter referred to as “the Facility”) located at 12758 235 St., Maple Ridge, BC, Canada. These activities may involve swinging bats, hitting baseballs, pitching, catching, and other physical exertion.

Assumption of Risk
I acknowledge that participation in baseball activities, including the use of the batting cage and private lessons, involves inherent risks, including but not limited to bodily injury, permanent disability, or death. These risks may arise from the use of equipment, weather conditions (e.g., rain, wind, or sun exposure), the actions of other participants, or my own physical condition. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Facility, its owners, employees, or agents.

Release of Liability
In consideration of being permitted to use the batting cage and/or participate in private baseball lessons, I hereby release, waive, discharge, and agree not to sue The Front Yard Baseball, its owners, coaches, employees, or agents (collectively, “the Released Parties”) from any and all liability for any injury, loss, or damage, including personal injury, property damage, or death, whether caused by negligence or otherwise, arising out of my participation in these activities.

Indemnification
I agree to indemnify and hold harmless the Released Parties from any claims, demands, or causes of action brought by myself, my minor child/ward, or any third party arising from my participation in the activities at the Facility.

Medical Authorization
I certify that I (or my minor child/ward) am physically fit to participate in baseball activities and have not been advised otherwise by a qualified medical professional. In the event of an injury or medical emergency, I authorize the Facility to seek and obtain medical treatment on my behalf if I am unable to consent at the time.

Equipment and Facility Use
I agree to use all equipment, including bats, balls, helmets, and the batting cage, in a safe and responsible manner as instructed by the Facility’s staff. I understand that the batting cage is located outdoors and subject to natural elements, and I assume all risks associated with weather-related conditions.

-Batters MUST wear helmets inside tunnels at all times​

-All balls must be picked up and returned to their proper place after use.

-Batters must wear appropriate shoes. Turf shoes allowed, no cleats.​​

Governing Law
This waiver shall be governed by and construed in accordance with the laws of the Province of British Columbia and the federal laws of Canada applicable therein.

Acknowledgment
I have read this waiver and release form carefully and fully understand its contents. I am aware that by signing this document, I am giving up substantial legal rights, including the right to sue. I sign it freely and voluntarily without any inducement.​​​​​
